The association between chronic heart failure (CHF) and iron deficiency (ID) anemia is frequently observed in current medical practice and has a negative prognostic impact, reducing patients’ exercise capacity. Moreover, anemia is common in patients with chronic kidney disease (CKD) and CHF, forming an association known as cardio-renal anemia syndrome (CRAS), which significantly increases the risk of mortality. This article aims to present the impact of ID in CHF patients and the benefits of iron supplementation in CHF, based on the relationship between functional and absolute ID in CHF. A complex pathophysiological interaction is described, including decreased erythropoietin levels and activation of the renin-angiotensin-aldosterone system. These mechanisms exacerbate anemia, CHF, and CKD, worsening patient outcomes.
