Premature rupture of membranes (PROM) at term occurs in 8–10% of pregnancies and remains one of the leading causes of maternal and neonatal complications. The choice of optimal management strategy in full-term pregnancies (≥37 weeks) is of particular importance, as both delayed delivery and aggressive labor induction can be associated with adverse outcomes. This article presents a contemporary overview of the etiopathogenesis of PROM, including infectious, mechanical, genetic, and metabolic factors. Current approaches to the management of PROM are discussed, including methods for assessing cervical ripeness and various labor induction techniques such as balloon catheters, prostaglandins E1 and E2.The article summarizes data from clinical studies, as well as international and national guidelines, emphasizing the need for an individualized approach in choosing the method of induction to minimize the risk of infectious and perinatal complications.
