Orthodontic treatment plays a vital role in addressing dental and skeletal malocclusions to enhance both functional occlusion and facial aesthetics. However, some cases—classified as irreversible due to their complexity—pose significant challenges to conventional treatment methods. These include skeletal anomalies, periodontal compromises, and severe malocclusions. To address these challenges, this study evaluates the clinical outcomes and patient satisfaction associated with individualized treatment programs compared to traditional orthodontic approaches. Conducted as a 12-month longitudinal observational cohort study at a specialized orthodontic clinic, the research included 120 patients aged 18–50 with irreversible orthodontic conditions. Participants were divided into two groups: Group A received individualized treatment guided by digital cephalometric analysis and AI-assisted planning, while Group B underwent standard conventional treatment. Clinical assessments included pre- and post-treatment cephalometric analysis, occlusal evaluations, and patient satisfaction surveys.
