Improving the treatment of facial-orbital fractures based on a clinical algorithm

Fractures of the zygomatico-orbital complex (ZOC) are among the most common injuries of the midface and are accompanied by pronounced functional and aesthetic disorders. Diagnostic errors, delayed repositioning, and inadequate reconstruction of orbital walls lead to persistent diplopia, enophthalmos, and facial asymmetry. The aim of the study was to develop and clinically evaluate an algorithmic approach to treating patients with ZOC fractures, taking into account the degree of bone fragment displacement, involvement of orbital structures, and timing of medical consultation. A clinical observation of 84 patients was conducted, divided into two groups: control (traditional tactics) and main (algorithmic approach). In the main group, a statistically significant decrease in the frequency of late complications was noted: diplopia (from 16.7% to 4.8%), enophthalmos (from 19.0% to 7.1%), and pronounced facial asymmetry (from 23.8% to 9.5%). The use of the algorithm made it possible to standardize the stages of examination and surgical treatment, optimize the choice of surgical approaches and osteosynthesis points, and improve functional outcomes.
