Otitis media, particularly its exudative variant, ranks among the most frequently diagnosed conditions in pediatric populations. In infants and young children, it often progresses with minimal or no clinical symptoms, which increases the likelihood of delayed detection and may result in hearing impairment. Even mild auditory dysfunction in such cases can have a pronounced impact on a child's speech acquisition, auditory processing, and overall cognitive development. Pediatricians are typically the first healthcare professionals to identify the initial manifestations of exudative otitis media in routine clinical settings. For this reason, it is essential that they possess a comprehensive understanding of the condition, including its predominant etiological factors and risk determinants. The initial section of the literature review explores prevailing theories surrounding the pathogenesis of exudative otitis media, highlighting the involvement of viral and bacterial agents, auditory tube dysfunction due to nasopharyngeal masses, as well as the influence of craniofacial malformations (notably those associated with chromosomal syndromes), allergic conditions, rhinosinusitis, and adenoid hypertrophy.
