Ketogenic diet and intermittent fasting in dentistry: a review of their effects on the oral microbiome, salivary metabolism, and cariogenicity

The oral cavity represents a complex ecological system, where the microbiome, saliva composition, and dietary habits interact closely to maintain or disrupt oral health. In recent years, metabolic strategies such as the ketogenic diet (KD) and intermittent fasting (IF) have attracted research attention due to their pronounced effects on systemic metabolism, inflammatory processes, and gut microbiota. However, the impact of these dietary approaches on the oral microbiome, salivary metabolism, and cariogenic potential remains insufficiently studied.This review summarizes current data on the relationship between KD and IF and the biochemical and microbiological environment of the oral cavity. Analysis shows that both dietary strategies can alter the microbial ecosystem, the rate and composition of saliva secretion, buffering capacity, and pH, thereby influencing caries risk. Carbohydrate restriction in KD may reduce the activity of acid-producing bacteria; however, decreased saliva secretion and changes in its composition may counteract this effect. Similarly, fasting phases in IF are accompanied by fluctuations in salivary and immune parameters, which can temporarily alter microbiota balance. Comprehensive clinical-metabolomic and microbiome studies are needed to clarify these mechanisms and assess the potential of KD and IF in the prevention of dental diseases.
