REGIONAL MELANOMA METASTASIS AND THEIR TREATMENT

Objective: To study the features of regional metastasis in melanoma and the optimal approach to their treatment. Methods: The study included data from 61 patients treated with a diagnosis of skin melanoma for the period from 2000 to 2021 inclusive. Results: In the patients with skin melanoma with regional metastases observed by us, the primary tumor was more often localized on the leg (40.1%) and trunk (30%), less often on the head, face (10.5%) and arm (10.1%). Metastases without identified primary focus (T0) were found in 5.6% of cases. Of the 47 patients, 30 died in the first year, and 9 died in the second year. In deceased patients, metastases to internal organs prevailed: lungs, liver, brain. It was also noted the defeat of metastases of the lymph nodes, skin, subcutaneous tissue. The high mortality rate in the first 2 years after treatment (15 out of 47) suggests that the majority of patients admitted for treatment with regional metastases already had latent metastases to the internal organs. A five-year period after treatment of melanoma of the skin with regianary metases survived 8 (14%) of 47 patients. Conclusions. The number of patients with advanced forms does not decrease and their number remains high. Surgical, combined and complex methods of treatment of patients with melanoma with regional metastases do not give a significant difference. Conservative methods are purely palliative and do not significantly increase the life expectancy of patients. Early diagnosis and early treatment is the only way to improve the results of skin melanoma therapy.
