MODERN STRATEGY OF CONSERVATIVE THERAPY OF DEGENERATIVE CERVICAL MYELOPATHY: NEUROPRECOVERY, NEUROREGENERATION AND NEUROMODULATION (LITERATURE REVIEW)

Annotatsiya
Degenerative cervical myelopathy (DCM) is the most common cause of spinal cord compression and its subsequent degeneration in adults. Despite the fact that surgical decompression is the main method of treatment for severe degrees of compression, conservative therapy plays an important role in the early stages of the disease, as well as in patients with contraindications to surgery or mild compression. The article discusses a modern strategy of conservative treatment of DCM, focusing on the principles of neuroprotection, neuroregeneration and neuromodulation. The evidence base and mechanisms of action of various approaches are analyzed, including medications (neuroprotectors, metabolic activators), physiotherapy, kinesitherapy and the latest methods of non-invasive neuromodulation (transcranial magnetic and electrical stimulation). The possibilities of slowing down the progression of the disease, improving the neurological status and quality of life of patients with the help of complex conservative strategies aimed at maintaining the function of the spinal cord and modulating its excitability are discussed.
