. This study is aimed at a comprehensive assessment of the clinical effectiveness and immune tolerance of using a cheek fat body patch during palatoplasty in children with congenital cleft palate. The purpose of the study was to conduct a comparative analysis of the effectiveness and safety of this method based on healing indicators, the frequency of postoperative complications, as well as immunological and morphofunctional markers of inflammation and regeneration. The study included 72 children aged 9 months to 2 years, divided into two groups: the main group (n=38), in which a cheek fat patch was used, and the control group (n=34), in which the traditional defect closure technique was used. Comparative analysis showed that in patients of the main group, the timing of epithelialization was shorter by an average of 4-5 days, the frequency of fistula formation and purulent complications was reduced by more than 2 times. Systemic inflammation indicators (IL-6 and TNF-α levels) in the main group were statistically significantly lower, and the CD4+/CD8+ index reflected a more balanced immune status. Immunohistochemical examination of tissues showed an increase in VEGF and TGF-β expression in patients of the main group, which indicates enhanced angiogenesis and reparative activity. Thus, applying a cheek fat body patch not only ensures reliable closure of the defect but also contributes to a reduction in the inflammatory load and accelerated tissue restoration. The obtained data confirm the clinical effectiveness and immunological safety of the methodology, justifying its inclusion in the standards for surgical treatment of congenital cleft palate in children.
