Clinical and dental assessment of patients with mitral heart disease

To assess dental status in patients with mitral valve disease, identify oral parameters linked to cardiac decompensation, and estimate the clinical value of a stepwise dental routing algorithm. Materials and methods. The study included 138 participants: 42 controls, 48 patients with compensated mitral valve disease, and 48 with decompensated disease. Dental examination comprised DMFT, OHI-S, SBI, CPI, probing pocket depth, clinical attachment loss, number of retained teeth, xerostomia score, unstimulated salivary flow, and odontogenic infectious foci. Results. The decompensated group had higher OHI-S, CPI, PPD, and CAL values, fewer remaining teeth, more severe xerostomia, and the greatest need for preoperative oral sanitation. Functional class by NYHA was directly correlated with key dental and periodontal indicators. An integrated risk score for high oral infectious risk yielded an AUC of 0.891. Conclusion. Worsening hemodynamic status in mitral valve disease is accompanied by a progressive deterioration of oral and periodontal health, which supports early dental screening and targeted sanitation before invasive cardiologic or cardiac surgical care.
