Current approaches to purulent maxillofacial infections: source control, antimicrobial therapy, and emerging technologies

Background: Purulent odontogenic and non-odontogenic maxillofacial infections remain clinically relevant because of the risk of rapid spread through fascial spaces with potential airway compromise, sepsis, cervicofacial necrotizing fasciitis, and secondary osteomyelitis. Aim: To summarize current diagnostic and therapeutic principles for purulent maxillofacial infections and to appraise the role of minimally invasive and molecular diagnostic technologies. Materials and Methods: A narrative review of the literature (PubMed/Scopus, 2010–2025) was performed, focusing on odontogenic infections, deep neck space infections, and major complications (necrotizing infections and osteomyelitis). Evidence on airway management, imaging, surgical source control, antimicrobial therapy, and adjunctive treatments was synthesized. Results: Reproducible determinants of favorable outcomes include early severity and comorbidity assessment, contrast-enhanced CT when deep space involvement is suspected, timely elimination of the odontogenic focus with incision/drainage of collections, and rational empiric antimicrobial therapy followed by targeted de-escalation. Emerging options include ultrasound-guided percutaneous drainage in selected cases, enhanced microbiological workup (MALDI-TOF, 16S/NGS) for polymicrobial/anaerobic infections, and negative pressure wound therapy or hyperbaric oxygen as adjuncts in necrotizing disease when clinically justified. Conclusion: Management should prioritize safe airway control and early source control; innovations are best integrated through strict patient selection and critical appraisal of the evidence base.
