Modern principles of treatment for oral lichen planus

There is currently a considerable arsenal of treatments for OLP, with therapies varying in efficacy and safety. Topical corticosteroids are most widely used as «first-line therapy», with topical application of corticosteroids as the primary therapy. Because of side effects with prolonged use, systemic use of corticosteroids and immunosuppressants is limited. In addition to corticosteroids also prescribe treatment with immunosuppressants, retinoids, low-intensity laser therapy, photodynamic therapy, phytotherapy. Effective in symptom control and clinical resolution, topical calcineurin inhibitors have side effects including burning and dysgeusia. In patients torpid to standard therapy platelet concentrates are effective. The therapeutic horizons for the treatment of OLP are greatly expanded with systemic therapy, immunosuppressants, retinoids, laser therapy, photodynamic therapy, vitamin D, biologic drugs and Janus kinase inhibitors. Therapeutic strategies need to be tailored to the individual patient and the clinical course of the disease. Significant side effects of corticosteroids and insufficient effectiveness of auxiliary therapy determine the search and introduction of new drugs.
