The Relationship Between Geometric Types of Left Ventricular Remodeling and Uromodulin Levels in Cardiorenal Syndrome Associated with Chronic Heart Failure

The article presents a comprehensive assessment of the relationship between geometric types of left ventricular remodeling and uromodulin levels in cardiorenal syndrome (CRS) associated with chronic heart failure (CHF). The study included 240 participants: Group 1 — with CRS (n=100), Group 2 — CHF without CRS (n=100), Group 3 — control (n=40). In the CRS group, LVMI was 142.5±3.2 g/m² (without CRS: 108.7±2.6; control: 84.3±2.1), relative wall thickness (RWT) — 0.48±0.01. LVH was identified in 90% of CRS patients: eccentric hypertrophy — 55%, concentric hypertrophy — 28%, concentric remodeling — 12%, normal geometry — 5%. Grade III diastolic dysfunction was found in 50% of CRS patients. Uromodulin levels strongly depended on remodeling type: normal geometry — 92.5±3.2 ng/ml, concentric remodeling — 75.3±2.5, concentric hypertrophy — 62.4±1.9, eccentric hypertrophy — 48.6±1.7 ng/ml (p<0.001). A strong inverse correlation was found between uromodulin and LVMI (r=−0.71, p<0.001). 4 figures and 2 tables are presented.
