COMPARISON OF DIFFERENT METHODS OF DETECTION OF ISCHEMIA AND MYOCARDIAL VIABILITY IN ISCHEMIC HEART DISEASE

Therapeutic, pharmacological or surgical interventions in any disease should be aimed at improving the prognosis in terms of mortality, symptoms or quality of life. Therefore, coronary revascularization, whether by bypass or percutaneous coronary intervention, should be used only if the available data suggest that patients will benefit from one of these three options. Therefore, the patient should not only undergo a thorough anatomical examination, but also always undergo a clinical and functional assessment. Currently, the most preferred method in nuclear cardiology laboratories is gated SPECT. Simultaneous assessment of myocardial perfusion as well as global and regional contractility is possible by obtaining isotope myocardial perfusion images synchronized with the electrocardiogram and gated SPECT. The effectiveness of this method for assessing viability was studied by determining contractile reserve during dobutamine infusion at a dose of 10 mcg/kg/min, which gave a sensitivity of 96% and a specificity of 78%.
