Polycystic ovary syndrome (PCOS) is a frequent condition in women of reproductive age. PCOS include such symptoms as menstrual dysfunction, hyperandrogenism, and polycystic ovary morphology. Insulin resistance is at the heart of hormonal and metabolic changes. Phenotypically, obesity PCOS and lean PCOS are distinguished. The purpose of this review is to identify the main characteristics and to identify ways of diagnosing and treating lean people with PCOS based on new research. RESULTS: There were significant changes in hormonal-metabolic outcomes in lean individuals with PCOS compared to controls. At the same time, when comparing lean PCOS and obese PCOS, no significant difference was found. Insulin resistance occurs in both lean PCOS and obese PCOS. Treatment goals were weight maintenance, treatment of anovulation with insulin sensitizers, antiadrogen therapy for acne, and hirsutism. Conclusion. To confirm the results, further additional studies are needed to improve diagnostic and treatment algorithms. This document help improve the management of lean women with PCOS.
