FEATURES OF CARDIOVASCULAR REMODELING AND MODERN APPROACHES TO THERAPY OPTIMIZATION IN PATIENTS WITH HYPERTENSION ASSOCIATED WITH TYPE 2 DIABETES

Abstract
Relevance: The combination of arterial hypertension (AH) and type 2 diabetes (T2D) significantly increases the risk of cardiovascular complications, especially in ecologically challenging regions like Khorezm, Uzbekistan. Purpose: To study the mechanisms of cardiovascular remodeling, identify clinical-functional markers, and assess the efficacy of pathogenetically justified therapy. Materials and Methods: A prospective study included 42 patients aged 41-70. ECG, Holter monitoring, echocardiography, ABPM, and laboratory methods were applied. Treatment included empagliflozin (Diampa), perindopril (Prestarium), and Allapinin. Results: Therapy achieved BP control in 78.6% of patients, reduced left ventricular hypertrophy, improved LV ejection fraction, decreased rhythm disturbances, and improved renal function. Conclusion: Combined therapy effectively reduces cardiovascular remodeling severity and complication risks in patients with comorbid AH and T2D.
