Patient registries following acute myocardial infarction (AMI) and stroke are a key tool for quality control, real-world clinical practice research, and secondary prevention management. This article examines international registry models (SWEDEHEART, ACTION Registry–GWTG, MINAP, Get With The Guidelines–Stroke, Riksstroke/SSNAP, etc.), their strengths and limitations, and the experience of Uzbekistan (national and regional ACS/AMI and stroke registries). Approaches to data standardization and a practical roadmap for launching an outpatient registry for post-AMI and post-stroke in Uzbekistan are proposed, taking into account the ongoing digitalization of healthcare (DMED, Electronic Polyclinic, e-registry) and the specifics of the primary care system.
