Lipid metabolism and modern approaches to lipid-lowering therapy

Abstract
Lipid metabolism disorders are one of the leading risk factors for the development of atherosclerotic cardiovascular diseases and remain of high importance worldwide. Despite substantial advances in reducing low-density lipoprotein (LDL) cholesterol, many patients still have a residual cardiovascular risk associated with triglyceride-rich lipoproteins, remnant cholesterol, and Lp (a). This review summarizes current information on lipid metabolism, including exogenous and endogenous lipoprotein transport pathways, as well as modern strategies for lipid-lowering therapy, in particular drugs agents targeting PCSK9, ANGPTL3, ApoC III and LP(a). Special attention is paid to promising technologies based on small interfering RNAs, antisense oligonucleotides, and CRISP/Cas9 genomic editing. The collected data indicate that an deeper understanding of the mechanisms of lipid metabolism contributes to the development of innovative treatment methods that can more effectively control dyslipidemia and reduce the risk of cardiovascular complications.
