RESULTS OF DEFECT RECONSTRUCTION WITH A PECTORAL FLAP IN SURGICAL TREATMENT OF ORAL CANCER

Аннотация
Objective: to improve the results of surgical treatment of oral cancer using a pectoral flap. Methods: Under our observation there were 35 patients with oral cancer who underwent reconstructive plastic surgery using a pectoral flap in the Bukhara regional branch of the RSNPMCOR and R of the Ministry of Health of the Republic of Uzbekistan from 2014 to 2018. Of the 35 patients, there were 21 men (60%) and 4 women (11.4%). The age of the patients ranged from 30 to 70 years, the average age was 54.2 years. The most common disease occurred at the age of 50-59 years - 12 (34.2%) and 60-69 years - 10 (35.0%). Results: the results of combined treatment: out of 35 patients, 13 (80%) patients had a decrease in edema by 25% after surgery, in 18 (51%) patients, a decrease in edema by 50% after surgery, in 4 (11.4%) patients, a decrease 100% edema after surgery. Postoperative complications were not detected in 26(74%) patients, erythema and itching were detected in 6(17%), edema was detected in 1(3%), itching was detected in 1(3%), and a psychological disorder was detected in 1(3%). Of the 35 patients, 17 (49%) had a primary tumor recurrence and 18 (51%) patients had no primary recurrence. The degree of tumor differentiation was G1 in 15(43%) patients, G2 in 15(43%) patients, and G3 in 5(14%) patients. All patients underwent combined operations (excision and vascular plasty). Functional results of treatment: restoration of chewing in 6 (17%) patients, restoration of swallowing in 19 (54%) patients, restoration of swallowing + speech in 1 (3%) patient, restoration of chewing + swallowing + speech in 1 (3%) patient, recovery speech in 8 (23%) patients. Conclusions. The results of defect reconstruction with a pectoral flap in the surgical treatment of oral cancer are improved compared to the results of complex treatment without reconstructive plastic surgery.
