ETIOPATOGENETIC AND CLINICAL NEUROLOGICAL FEATURES OF ISCHEMIC STROKE IN YOUNG PEOPLE DEPENDING ON HETEROGENEITY

Purpose of the study: To study the etiopathogenetic and clinical - neurological features of ischemic stroke in young people depending on heterogeneity. Materials and methods. The study was conducted from 2018 to 2022. 133 patients with ischemic stroke in the acute period were examined, including 78 (58.6%) men and 55 (41.4%) women. All patients were divided depending on the age category: group I (main) - 83 patients aged 22-44 years (mean age 38.71±6.5 years; men - 41 (49.4%), women - 42 (50 .6%)), II (comparison) group - 50 patients older than 60 years (mean age 66.34±5.01 years; men - 37 (74%), women 13 - (26%)). Research results. When comparing the etiological factors in both groups, it was found that smoking and alcohol abuse, heart defects and kidney pathology occurred in young people with a high frequency. A comparative analysis of the distribution of patients by pathogenetic type showed that cardioembolic, lacunar and hemodynamic subtypes prevailed in the main group. In 12% of all cases, the onset of a stroke began with a convulsive syndrome. Clinical examination in the main group revealed the predominance of stem, motor, speech, sensory, cerebellar disorders, as well as the presence of bulbar and alternative syndromes. A comparative analysis of the neurological and functional status in young people according to the NIHSS scale revealed mild and severe neurological deficits, in isolated cases severe neurological disorders. Conclusion. The presented results made it possible to identify the main etiopathogenetic and clinical and neurological features of ischemic stroke in young people, depending on the pathogenetic subtype and etiological factor.
