Abstract
Aim of the study: study of the clinical features of the course and determination of the effectiveness of various surgical methods for the treatment of parasagittal meningiomas of the brain. Material and methods. The study involved 72 patients with meningiomas of the cerebral hemispheres, of which 38 patients were under their own supervision and 34 - the analysis of materials based on archival histories of the disease was carried out. 80% of patients were subjected to surgery. Research results. In the clinical course of BPM, one of the early symptoms of cerebral meningiomas is headache. Vomiting had specific signs, namely, it occurred regardless of food intake, sometimes in the morning. Mental disorders developed gradually with other cerebral symptoms as a result of an increase in the phenomenon of intracranial hypertension, edema and hypoxia of the brain. Decreased criticism was found in 7% of patients. Paresis in the extremities was observed in 40 (55.5%) patients, who encountered right hemiparesis in 17 (23.6%), left hemiparesis in 19 (26.3%), monoparesis in 3 (4.1%) and tetraparesis (1, 4%). BPM are often localized in the anterior third of the sagittal sinus and olfactory fossa. In 79.3% of cases, the neoplasm was removed totally (Block resection), in 5.7% - subtotally, in 7.3% of patients - partial removal, in 1.8% of cases a biopsy was required. There were contraindications of the cardiovascular system. 5.9% of patients did not undergo surgical treatment. Conclusion. Determination of the above regularities of the clinical course and timely application of modern surgical methods for treating patients with BPM contribute to improving treatment results and preventing various complications and disability.
