IMPROVEMENT OF THE SURGICAL APPROACH FOR CYSTIC LESIONS OF THE MAXILLARY SINUS

One of the leading places in the list of chronic pathologies of the nose and paranasal nasal cavities is occupied by chronic cystic lesion of the maxillary sinus (CKDVS). There is still no consensus on the modern approach to the diagnosis and treatment of cysts. In case of cystic lesions of the maxillary sinus, choosing the right treatment is one of the urgent problems of modern rhinosurgery. The purpose of our study was to substantiate the choice of a differential approach to surgical treatment of maxillary sinus cysts. The basis of our study was 153 patients with cystic lesions of the maxillary sinus, who were divided into 2 groups depending on the presence of pathological changes in the osteomeatal complex. The study began with an otolaryngological examination, which consisted of endoscopic rhinoscopy, pharyngoscopy, otoscopy and laryngoscopy, as well as computed tomography of the paranasal sinuses of the nose. Surgical treatment to remove the maxillary sinus cyst was performed in one and two stages, as well as using three approaches, depending on the location of the cyst in the sinus. At the end of the surgery, the operated cavities were examined and the dynamics of clinical manifestations of the postoperative inflammatory process and the development of complications at different times of the postoperative period were noted. When using the differential endoscopic method of cyst removal, the quality of treatment of sinus cysts of the upper jaw is significantly improved, the severity and number of postoperative complications are reduced.
