The article presents a description and scientific substantiation of cytologic aspects of confirming the stages of chronic exudative otitis media (CEMO). The pathophysiological mechanisms leading to the formation of exudate in the middle ear cavity are presented. The nature of cellular landscape changes depends on the stage of the disease. Based on the study, cytograms reflecting a certain stage of the disease were analyzed. Each type of cytogram allows a reliable assessment of the degree of dystrophic changes in the mucous membrane of the tympanic cavity. The proposed cytologic study of exudate in CESO allows to determine the type of inflammatory process in the tympanic cavity, and also helps to determine the final stage of the process according to the morphofunctional classification of N.S. Dmitriev et al. (1996), At present, cytologic classification of exudate in CESO has not been developed depending on the stage of the disease, which complicates clinical and instrumental diagnosis of the features of the disease in question. The possibility to estimate the state of local immunity (cytological study of middle ear secretion) allows to predict the clinical course of the disease and then to choose the optimal individualized tactics of therapeutic measures during treatment in inpatient and outpatient conditions.
