MODERN METHODS OF VISUALIZATION OF GALLBLADDER DISEASES

Benign gallbladder diseases typically present with intraluminal lesions and localized or diffuse wall thickening. Intraluminal lesions of the gallbladder include gallstones, cholesterol polyps, adenomas, or biliary sludge. Additionally, the polypoid form of gallbladder cancer must be ruled out. Polyp size, stalk width, and enhancement intensity on contrast-enhanced ultrasound, as well as the degree of diffusion restriction, may assist in differentiating cholesterol polyps and adenomas from gallbladder cancer. Localized gallbladder wall thickening is mainly associated with segmental or focal adenomyomatosis, although infiltrative cancer may present similarly. Identification of Rokitansky-Aschoff sinuses is crucial for diagnosing adenomyomatosis. The layered pattern, degree of contrast enhancement, and integrity of the wall are key imaging features that help distinguish benign thickening from gallbladder cancer. High-frequency ultrasound is especially useful for analyzing the layering of the gallbladder wall. Diffuse thickening of the wall is frequently seen in inflammatory processes of the gallbladder. However, it is important to check for coexisting cancer in cases of acute cholecystitis. Ultrasound alone is limited in evaluating complicated forms of cholecystitis and often requires additional computed tomography. In chronic cholecystitis, preservation of a two-layered wall and weak wall enhancement are diagnostic clues to exclude malignancy. Magnetic resonance imaging combined with diffusion-weighted imaging helps to differentiate xanthogranulomatous cholecystitis from gallbladder cancer by identifying the presence of fat and the degree of diffusion restriction. Such distinctions require familiarity with typical imaging features of various gallbladder diseases and an understanding of the roles different imaging modalities play in its evaluation.
