BIOMECHANICAL INSIGHTS INTO MDCO FOR FLATFOOT CORRECTION

Medial Displacement Calcaneal Osteotomy (MDCO) is increasingly used to correct flatfoot deformities. This study investigates the biomechanical effects of MDCO on foot function, including changes in plantar fascia strain, arch stability, joint pressure, and gait patterns. Methods: A systematic review of 27 studies was conducted, analyzing cadaveric, biomechanical model, and clinical data. Parameters such as plantar fascia strain, medial arch dynamics, and joint pressures were extracted and statistically summarized. Results: MDCO was found to significantly reduce plantar fascia strain, normalize excessive joint pressures, and improve medial arch stability. Additionally, specific gait parameters showed positive modifications postoperatively. Discussion: The findings support MDCO as an effective surgical intervention for flatfoot correction by optimizing force distribution and joint mechanics. However, some studies reported adverse effects on toe function and potential risks for ankle arthritis. Further research is needed to refine patient selection criteria and long-term outcomes.
