Аннотатсия

The article focuses on unresolved issues of medication‑related osteonecrosis of the jaw (MRONJ) in patients with bone metastases from prostate cancer receiving bisphosphonate therapy. Current concepts of the role of antiresorptive treatment in the management of skeletal metastases are summarized, and key stages in the pathogenesis of MRONJ under prolonged bisphosphonate exposure combined with androgen deprivation and systemic anticancer therapy are discussed. Particular attention is paid to risk factors specific to the prostate cancer population, including advanced age, multimorbidity, duration and intensity of antiresorptive therapy, poor oral health, and traumatic dental procedures. Diagnostic challenges are analyzed, such as early detection and differential diagnosis of jaw lesions in oncologic patients. Existing conservative and surgical treatment strategies for MRONJ are reviewed, with emphasis on controversial issues regarding the extent of surgery and the management of antiresorptive therapy once osteonecrosis has developed. The concluding section outlines the major unresolved problems and highlights future research directions, including the need for individualized risk prediction models and prevention algorithms tailored to patients with metastatic prostate cancer.
