Objective. To comparatively evaluate the clinical effectiveness, safety, and clinical-economic advantages of the Sentimag + Magtrace magnetic navigation system for sentinel lymph node detection in stage II–III breast cancer versus standard axillary lymph node dissection. Materials and Methods. A retrospective comparative study was conducted. Data from 100 patients with morphologically confirmed stage II–III breast cancer treated at the Republican Specialized Scientific and Practical Medical Center of Oncology and Radiology were analyzed. Patients were divided into two groups: Group I (n=50) — sentinel lymph node biopsy using the Sentimag system; Group II (n=50) — standard axillary lymph node dissection. Intraoperative parameters, length of hospital stay, postoperative complications, and economic indicators were assessed. Results. The detection rate of sentinel lymph nodes in the Sentimag group was 100%. The mean number of removed lymph nodes was 1–3, compared to ≥10 in the standard group. A significant reduction was observed in operative time (55 min vs 95 min), length of hospital stay (10 vs 15 bed-days), and postoperative complication rate (6.0% vs 60.0%) in the Sentimag group. Axillary lymph node dissection was avoided in 68.0% of patients. Economic analysis demonstrated savings of 1,125,000 UZS per patient. Conclusion. The Sentimag + Magtrace system demonstrates high accuracy, minimal invasiveness, safety, and cost-effectiveness in sentinel lymph node detection in breast cancer. The method reduces complications associated with axillary lymph node dissection and supports a personalized surgical approach, making it suitable for implementation in clinical practice.
