The results of a quality of life study of 121 patients with congenital spinal hernias of high localization, who were treated at the Republican specialized scientific- practical medical center of neurosurgery(RSSPMCN)in the period from 2013 to 2023, were studied. Two scales were used to assess the quality of life: the non-specific Lansky scale and a specially developed scale for assessing the quality of life of children with spinal hernias. This adapted scale made it possible to objectively assess the results of treatment and the level of comfort of patients. The introduction of a diagnostic and treatment algorithm also contributed to the improvement of surgical results, which was expressed in a decrease in neurological disorders and an increase in the quality of life of patients. Purpose. To study the results of surgical treatment and assess the quality of life of children with congenital spinal hernias of cervico-thoracic localization. Materials. The work is based on the analysis of data from the examination and treatment of 121 patients with congenital spinal hernias who were at the RSSPMCN from 2013 to 2023. Results. All 121 children examined in our work underwent surgical interventions of various volumes and nature. The volume and sequence of reconstructive operations performed by us were determined taking into account the severity and dynamics of clinical manifestations, as well as taking into account the anatomical structure of the hernial sac and concomitant anomalies. Conclusion. An increase in the number of patients with a good quality of life after surgery was noted in the study group compared to the control: children with a good quality of life before surgery amounted to 6.6% in the study group and 15.0% in the control group (p<0.01). The developed algorithm for the management of patients with highly localized spinal hernias made it possible to differentiate the approach to surgical treatment, and, as a result, improve the results of treatment and the quality of life of patients.
