COMPARATIVE CHARACTERISTICS OF AUTONOMIC DYSFUNCTION AND 24-HOUR BLOOD PRESSURE VARIABILITY IN PATIENTS WITH TRANSIENT ISCHEMIC ATTACK

Abstract

Objective. To assess the state of the autonomic nervous system in patients with transient ischemic attack (TIA) using the VNS-31 questionnaire and 24-hour ambulatory blood pressure monitoring (ABPM), and to evaluate the effect of comprehensive therapy on autonomic regulation and blood pressure variability. Materials and Methods. The study included 508 patients diagnosed with transient ischemic attack: the main group (n=231), the comparison group (n=145), and the parallel group (n=132). Autonomic dysfunction was assessed using the VNS-31 questionnaire on days 1–3, day 20, and at 3- and 6-month follow-up. Twenty-four-hour ambulatory blood pressure monitoring (ABPM) was used to evaluate the circadian blood pressure profile, nocturnal blood pressure dipping index, non-dipper and night-peaker patterns, and blood pressure variability. A p-value of <0.05 was considered statistically significant. Results. At baseline, all study groups demonstrated moderate to severe autonomic dysfunction (VNS-31 >60 points). In the main group, a significant reduction in symptom severity was observed from day 20 onward, with the VNS-31 score decreasing to 34.6±6.9 points after 6 months. In contrast, clinically significant manifestations of autonomic dysfunction persisted in both the comparison and parallel groups throughout the 6-month follow-up period. ABPM findings showed that patients residing in desert regions more frequently exhibited non-dipper and night-peaker blood pressure patterns, as well as increased blood pressure variability. Patients living in mountainous areas more commonly experienced hypotensive episodes, whereas those from lowland regions predominantly demonstrated a physiological nocturnal dipping pattern. Conclusion. In patients with transient ischemic attack, autonomic dysfunction is closely associated with blood pressure variability. The VNS-31 questionnaire and 24-hour ambulatory blood pressure monitoring are highly informative tools for assessing disease progression and treatment effectiveness. Comprehensive therapy significantly improves autonomic regulation and 24-hour hemodynamic parameters.
