SECONDARY STENOSIS OF THE CERVICAL SPINAL CANAL ASSOCIATED WITH COMPRESSIVE-ISCHEMIC MYELOPATHY OF THE SPINAL CORD: A CLINICAL CASE

This article analyzes a clinical case of secondary stenosis of the cervical spinal canal associated with compressive-ischemic myelopathy of the spinal cord. This pathology is related to narrowing of the spinal canal caused by degenerative-dystrophic changes, formation of disco-osteophytic complexes, intervertebral disc herniation, uncovertebral arthrosis, facet joint hypertrophy, and thickening of the ligamentous structures. Chronic mechanical compression of the spinal cord leads to impaired local microcirculation, venous congestion, disturbance of cerebrospinal fluid dynamics, and the development of compressive-ischemic injury. Clinically, the patient may present with pain in the cervical spine, numbness and decreased sensation in the upper extremities, muscle weakness, impairment of fine motor skills, gait instability, hyperreflexia, spasticity, and pathological pyramidal signs. The article highlights the diagnostic value of magnetic resonance imaging and computed tomography, the prognostic role of intramedullary T2-weighted hyperintensity, clinical-radiological correlation, and the criteria for selecting decompressive treatment tactics. The presented clinical case demonstrates that early detection of myelopathic signs in secondary cervical stenosis, accurate assessment of the degree of spinal cord compression, and timely selection of treatment strategy are essential for preventing persistent neurological deficit and patient disability.
