CLINICAL AND ANAMNESTIC PREDICTORS OF DRUG-RESISTANT EPILEPSY DEVELOPMENT IN ADULT PATIENTS

Drug-resistant epilepsy is one of the most challenging problems in modern neurology and epileptology due to persistent epileptic seizures, progressive cognitive and psychoemotional impairment, an increased risk of injury, social maladaptation, and premature mortality. A retrospective comparative single-center study was conducted and included 112 patients aged 18 to 70 years. The main group consisted of 64 patients with drug-resistant epilepsy, while the comparison group included 48 patients with stable medication-controlled seizures. The analysis included age at seizure onset, disease duration, epilepsy history, baseline seizure frequency and seizure polymorphism, response to the first antiseizure medication, history of perinatal pathology, febrile seizures, neuroinfection, traumatic brain injury, structural brain abnormalities, and epileptiform activity on electroencephalography. Independent predictors of drug resistance were lack of an adequate response to the first antiseizure medication, seizure frequency of at least four episodes per month before treatment initiation, the presence of a structural epileptogenic substrate on magnetic resonance imaging, and epilepsy onset before the age of 18 years. Based on the identified factors, a clinical and anamnestic risk score was proposed, with an area under the ROC curve of 0.86. The obtained results indicate the possibility of early identification of patients with an increased probability of drug resistance using accessible clinical and anamnestic parameters. The proposed model may be used for timely referral of patients to a specialized epileptology center and early consideration of alternative treatment strategies.
