THE ROLE OF MATRIX METALLOPROTEINASES (MMP-1, MMP-3, MMP-9) AND THEIR TISSUE INHIBITORS (TIMP) IN THE PATHOGENESIS OF INTERVERTEBRAL DISC DEGENERATION

Intervertebral disc (IVD) degeneration is a key link in the pathogenesis of discogenic back pain, disc herniation, and Modic endplate changes. The central role in the destruction of the disc extracellular matrix is played by the family of matrix metalloproteinases (MMPs) - zinc-dependent endopeptidases capable of cleaving collagen, aggrecan, and other matrix components. This review systematizes current data on the role of three key enzymes - collagenase MMP-1, stromelysin MMP-3, and gelatinase MMP-9 - in the initiation and progression of the degenerative cascade, as well as the significance of their endogenous inhibitors (TIMP-1–4) in maintaining the catabolic-anabolic balance of the matrix. Particular attention is paid to the relationship between the MMP/TIMP system and the formation of Modic endplate changes, and to the prospects of using these biomarkers in the personalized diagnosis and therapy of degenerative spinal diseases.
